How mapy learn about current traffic? If I have app installed on the phone, does it help in any way? Or I should have app opened and in navigation mode?

It depends in what region. I know that in CZ they definitely get it through official government site that tracks closures, roadworks, traffic from monitoring cars and counting gates…
In my case its Poland ;)
But its not bad
